Version 2021.01 of the Vaccination Component describes 2 document templates:

• The immunisation record (in CDA R2 level 3 format), which lists the vaccines administered to a patient, vaccination recommendations and refusals
• Vaccination history (in CDA R2 level 3 format) which consolidates, for a patient, all the vaccines given.
